Breast implants can be placed either in a pocket behind the breast tissue and above the pectoralis major muscle (muscle of the chest wall), or below the chest muscle. But is breast augmentation better done above or below the muscle?

The optimal placement for breast implants will vary for each patient, as it depends on a variety of factors including body type, implant type, and implant size. There are pros and cons to each approach.

Breast implants above the chest muscle (Sub-glandular placement)
Placing the breast implantson top of the chest muscle generally gives the breasts a round,less natural appearance. Surgery is somewhat easier with this method, and there is also less post-operative discomfort, since the chest muscle is not disturbed.

However, having sub-glandular placement may increase the risk of bottoming out, which is the downward migration of the implants below the inframammary crease. There is also a higher rate of capsular contracture (hardening of the breast implants), with sub-glandular placement.

Breast implants below the chest muscle (Sub-muscular placement)
Sub-muscular placement generally makes the breasts look more natural, as the implants are placed below both the breast tissue and chest muscle. Since the edges of the implants are concealed with this approach, it’s often preferred for women with a slim physique and very little breast tissue.
